EFFECT OF LOW INTENSITY PULSED ULTRASOUND ON CALLUS FORMATION IN TIBIAL DIAPHYSEAL FRACTURE - A REVIEW

Abstract
Management of tibial fracture is mainly conservative using cast or splint and/or surgical fixation
with reduction for immobilization. Lately intramedullary nailing (IMN) are used for effective immobilization
and proper healing. Due to limited soft tissue coverage over tibia complications like delayed union/non-union
are seen as compared to fractures of other bones. For faster results effective adjunctive therapies are explored.
Low intensity pulsed ultrasound (LIPUS) has been used as one of such adjunct therapy in fracture healing.
Finding effects of LIPUS was the prime objective of this review on fresh tibia shaft fracture treated with
intramedullary nail fixation. For this purpose electronic database (Medline, Cochrane and Google Scholar) from
1995 to 2020 was searched. Using ‘ultrasonic’, fracture healing, tibial, randomized control trials (RCT) as key
words. Among the selected studies there were heterogeneities due to which data could not be pooled. The mean
age of patients reported by these authors ranged from 35 to 39 years; LIPUS device was used with treatment
durations of 20 minutes/day with varying assessment intervals and follow-up. Assessment methods used were
radiographs (RUST 3/4 cortices), SF36PCS, health utility index, bone mineral or ALP levels, diagnostic
sonography and clinical examination (tenderness and weight bearing). From the review it was concluded that
LIPUS is an effective modality to shorten the healing time for fractures treated non operatively but no such
definitive statement can be make regarding fracture treated through IMN fixation. Further high quality
randomized control trials with larger number of patients and relatively uniform experimental setup be conducted
to resolve this issue.
