AN EVALUATION OF THE MORPHOLOGICAL SPECTRUM AND CLINICOPATHOLOGICAL OBSERVATIONS OF RENAL BIOPSY SPECIMENS IN VARIOUS RENAL DISEASES

Abstract
Background: Renal biopsy is a useful tool in the diagnosis of various renal parenchymal diseases.
This study was attempted to find the incidence of primary and secondary glomerulopathies in central India,
along with studying their morphological patterns and biochemical correlations in order to help clinicians get an
insight of treatment and prognosis. Materials and Methods: The present study on evaluation of the spectrum and
histopathological findings of percutaneous renal biopsy specimens in various renal diseases was carried out in
60 patients admitted in M.Y. Hospital, Indore. Biopsy was done under ultrasound guidance and morphological
evaluation was done by light microscopy. Statistical Analysis and Results: Maximum numbers of patients were
in the age group more than 40 years and most of them were males, with a Male: Female ratio of 3:1.Biochemical
alterations were mainly reduction of the blood urea nitrogen (BUN) and creatinine ratio (62%).
Hypercholesterolemia was seen in 57% cases. Maximum number of patients had mild proteinuria (42%).
Radiologically the kidney size was normal in most of the cases (83.3%). Light microscopic histopathological
findings were: the glomerular cellularity was increased in 55% cases and the least number of cases (6.7%) were
atrophic. Capillary basement membrane was prominently thickened in maximum number of cases (30%).
Bowmans capsule was found normal in fair number of cases (42%).There was significant mesangial
proliferation in maximum number of cases (33.3%).Tubules were found normal in most of the cases (70%).The
interstitium was found chronically inflamed in 93% cases. Blood vessels were found normal in maximum
number (42%) of cases. The commonest renal lesion was found to be Mesangio-proliferative
Glomerulonephritis (33%).The commonest and the only secondary glomerulopathy found was Amyloidosis
(18%). Conclusion: A conclusion on renal disease on the basis of light microscopy alone is not accurate without
immunoflorescence studies and electron microscopy. Our endeavor here is to evaluate renal disorders on the
available resources and help the nephrologists to have an idea of the epidemiology, clinical picture and light
microscopy findings, for deciding the treatment of the diseases.
