ROLE OF A NOVEL BIO ACTIVE MEMBRANE AND BONE GRAFT MATERIAL IN COMBINATION WITH PLATELET RICH FIBRIN TOWARDS PREDICTABLE HEALING OF PERIAPICAL LESIONS: A CASE SERIES

Abstract
INTRODUCTION: Periapical inflammatory lesion is a response of bone around the apex of tooth which develops as an aftermath of necrosis of the pulp tissue or extensive periodontal disease. Endodontic surgery is the last option that an endodontist has to successfully treat periapical pathosis. Advances in technology including specially designed instruments, improved root-end filling materials, superior quality bone graft materials, bioactive membranes and a thorough understanding of the biology of wound healing, have all contributed to a better outcome of endodontic surgery.
PRESENTATION OF CASE: Three case reports were chosen here with a purpose to evaluate the combined beneficial effect of platelet rich fibrin, bone graft, a bioactive membrane along with osteoconductive properties of biodentin used as root end filling material to treat cases with persistent periapical lesion. The patients have been assessed taking into consideration two parameters, firstly reduction in post operative pain and secondly radiographic evidence of bone healing.
DISCUSSION: Various regenerative therapies have been adopted like the use of bone graft and barrier membranes to enhance clinical and radiographic healing outcomes in the periapical defect area after degranulation of the lesion and different bio active materials being used for root end filling to achieve a more predictable success. Our case series here provides an insight into the rapid healing of large periapical lesions using BioGen mix bone graft along with platelet rich fibrin in combination with Bio Xclude membrane after retrograde filling of the root end using biodentin.
CONCLUSION: Successful healing achieved in all the three cases emphasizes the significance of using a combination of platelet rich fibrin, bone graft, bioactive membrane and a bioactive root end filling material to enhance radiographic healing outcome with decreased post-operative pain and present as the most dependable option for treating persistent periapical lesions.
