MATRIX METALLOPROTEINASES AS COLLAGEN MARKERS EXCHANGE WITH CONGESTIVE CARDIAC INSUFFICIENCY WITH AN INTERMEDIATE EJECTION FRACTION WITH PATIENTS WITH CORONARY HEART DISEASE AND METABOLIC SYNDROME

Abstract
The aim is to evaluate the peculiarities of matrix metalloproteinases (MMP) role as collagen markers exchange with heart failure with mid-range ejection fraction (HFmrEF) with patients with coronary heart disease (CAD) and metabolic syndrome (MS).
Material and Methods: 180 patients with HFmrEF (ejection fraction (EF) 40-49% according to Simpson) II functional class in the setting of CAD are included, all patients are divided into two groups: 1 subgroup - with MS, 2 subgroup without MS. According to the age it was assessed: middle age (52.26.5 years), old age (67.26.7 years), senile age (78.46.7 years). The analysis MMP-9, ng/ml MMP-9 ELISA (Bender Medsystems, Austria), tissue inhibitor metalloproteinas (TIMP-1) TIMP-1, ng/ml - Human TIMP-1 ELISA (Bender Medsystems, Austria) was conducted.
Results: The patients with HFmrEF of the middle age has the correlation MMP-9/TIMP-1 that were higher with the possession of MS in comparison with the group without MS in regard to the group without MS (∆ - 20%, р<0.05), old age (∆ -32%, р<0.01), senile age (∆ - 20%, р<0.05). Depending on the age the correlation MMP-9/TIMP-1 with patients with MS senile age was (∆ - 29%, р<0.001), than with old age and higher (∆ - 52%, р<0.001), with old age (∆ - 32%, р<0.001), than with middle age. Depending on the age without MS MMP-9/TIMP-1 with patients of the senile age is higher (∆ - 37%, р<0.001), than with old age and people of the middle age (∆ - 50%, р<0.001). With patients of middle age and older (∆ - 21%, р<0.01) the average dependence correlation is set between MMP-9 с TIMP-1 (r = 0.42: р˂0.05).
Conclusion: Age increment is the deprivation factor of fibrosis myocardium. MS deprives collagen exchange with the increase of age. The correlation MMP-9/TIMP-1 is the marker of fibrous tissue formation with patients with HFmrEF with MS and it is necessary to be controlled for this type of therapy.
