INNOVATIVE APPROACHES TO PATHOMORPHOLOGICAL VERIFICATION OF ACUTE APPENDICITIS.

Abstract
Objective: Development of innovative approaches in the diagnosis of acute appendicitis.
Methods: The study involved 76 children aged from 7 to 14 years with acute appendicitis. Children were examined. Clinical blood tests were performed upon their admission and at discharge. The appendix sections were studied by light microscopy with hematoxylin and eosin staining using a NikonTi light microscope. Scanning electron microscopy was performed using FE1 Quanta 200 3D and FE1 Quanta 600 FEG scanning microscopes (NT-MDT, Russia).
AFM was performed on a NTEGRAVita scanning probe microscope (NTEGRAVita) based on an OlympusIX-71 inverted optical microscope. Scanning was carried out in a moist chamber in the tapping mode using commercial NSG 01 Si or SiN cantilevers (NT-MDT, Russia). Continuous or intermittent contact modes were used. Computer processing and construction of AFM images (AFM images) was performed with NOVA (NT-MDT, Russia) and ImageAnalysis (NT-MDT, Russia) software.
Results: The AFM method revealed a number of structural features of blood cells in children with acute appendicitis, namely the predominance of cytoplasm over the nucleus in height, an increase in the number of plasmalemma microprotrusions, their base width and height, an increase in the diameter of neutrophils, erythrocytes and their central depressions, as well as the area, increasing the flattening coefficient of neutrophils. The low specificity of leukocytosis and a leftward shift of the leukocyte formula as markers of destructive appendicitis was revealed. In this case, an increased number of segmented white blood cells, lymphopenia, Ia.Ia. Kalf-Kalif leukocyte intoxication index are specific for destructive appendicitis.
Summary: the use of AFM of blood cells in combination with a number of criteria used in routine examination will significantly improve the accuracy and specificity of the diagnosis of destructive forms of acute appendicitis. The use of scanning electron microscopy in pathomorphological verification of the diagnosis provides not only high-resolution three-dimensional scans, but also enables histological rapid diagnostics within the first hour after the administration of the drug.
