FEATURES OF LEFT VENTRICULAR MYOCARDIAL REMODELING IN ELDERLY PATIENTS AFTER MYOCARDIAL INFARCTIONWITH ST SEGMENT ELEVATION (REVIEW).

Abstract
Coronary heart disease (CHD) is a leading cause of mortality among older people in Europe and the Russian Federation. One of its manifestations is myocardial infarction. This literature review addresses questions on the features of the course of ST elevated myocardial infarction (STEMI) in patients of older age groups. It has been shown that CHD is more severe and more often fatal. This is due to concomitant pathology, less adherence to therapy, and the duration of the disease. The mechanisms of physiological and pathological changes in the cardiovascular system in elderly patients are described in this review. This article presents new information about Structural and Functional changes in the heart with age. The mechanisms and features of adaptive myocardial remodeling in STEMI elderly patients are described. Changes in the humoral regulation of cardiac activity occur with an increase in the activity of the sympathetic nervous system in elderly people. Losing the number and decreasing the sensitivity of cardiac beta-adrenergic receptors requires an increase in the production of catecholamine. The external manifestation of these changes is an insufficient increase in heart rate, syncope, increased blood pressure, slower response to various stimuli, slow recovery after pharmacological tests. The research data on increasing the level of atrial natriuretic peptide factor in blood plasma during aging, while reducing the sensitivity of organ tissues to this hormone, are described. Modern data on elderly patients who have successfully performed primary percutaneous coronary intervention (PCI) are presented. Thus, several studies have been highlighted that patients with good coronary collaterals showed: normal myocardial perfusion myocardial perfusion, corresponding to grade 3 on the Myocardial Blush Grade (MBG) scale and a lower Killip class after primary PCI. So, the presented literature review highlights the features of structural and functional remodeling of the left ventricle (LV) in elderly patients who underwent STEMI. It was shown that revascularization promotes regression of structural and functional LV remodeling, a decrease in the 30-day level of general mortality, severity of hypertrophy, myocardial remodeling, improvement of diastolic function and an increase in ejection fraction.
Coronary heart disease (CHD) is a leading cause of mortality among older people in Europe and the Russian Federation. In addition, in patients of the older age groups, myocardial infarction with ST segment elevation (STEMI) is often preceded by heart failure. For elderly patients, both physiological and pathological changes in the cardiovascular system are characteristic. This article studies the structural and functional changes in the heart with age in terms of literature data. The features of myocardial remodeling in STEMI in elderly patients were studied.
