A PROSPECTIVE CLINICAL STUDY OF GLYCEMIC CONTROL IN WELL CONTROLLED TYPE II DIABETES MELLITUS PATIENTS SUBJECTED TO OPERATIVE PROCEDURES UNDER REGIONAL VERSUS GENERAL ANAESTHESIA

Abstract
Background - Diabetes mellitus (DM) is a disease characterized by hyperglycemia that results from defect in insulin secretion or action or both. The prevalence of type II diabetes mellitus is rapidly increasing all over the world, only 5 to 10% of all cases of diabetes are type 1 the rest belong to type 2 Diabetes. Type 2 diabetes is responsible for 90% of all cases of diabetes mellitus in the world.
This study is to evaluate and compare a better anaesthetic technique for precise maintenance of glycemic control between 100-180 mg/dl in a known type 2 diabetic patient on insulin, subjected to operative procedure under regional versus general anaesthesia.
Material And Methods-Total number of patients proposed for the study is 100 out of which 50 cases will be given general anaesthesia and 50 will be managed under regional anaesthesia according to patients willingness or alternately.
Results-The results of our study showed that patients undergoing GA showed significant rise in blood sugar level due to surgical stress, right from induction of anaesthesia with laryngoscopy and intubation, as against regional technique (NB) this significant rise in blood sugsr level not seen.
Conclusion- The intratracheal general anaesthesia as conceived is not an ideal anaesthetic technique as evidenced by its failure to control the metabolic component of glycemic control in type 2 diabetes patients as against a well conducted regional block (neuraxial block).
