NEW TECHNOLOGIES FOR RECONSTRUCTION OF THE LOWER JAW DEFECTS WITH THE USE OF INDIVIDUAL CONSTRUCTIONS AND TENSION-FREE MATERIALS AS AN ARTIFICIAL PERIOSTEUM.

Abstract
Reconstruction of the lower jaw after partial or complete resection is a requirement for the restoration of normal
facial aesthetics, articulation and chewing function.The lower jaw resection and endoprosthetics are complex and
demanding surgical interventions that determine the patient's quality of life for many years. These interventions are
most frequently performed for neoplasms of the mouth floor (38%) and the tongue (13%) [1]. The remaining
surgeries are for the consequences of trauma and osteonecrosis developed as a result of osteomyelitis. A significant
share in the structure of osteonecrosis is gained by bisphosphonate osteonecrosis, which is found in 0.8% -18.6% of
drug-injecting addicts[2,3].
Elimination of the lower jaw defects that result from injuries and bone inflammatory diseases, as well as
surgical interventions for tumor processes, is an important medical and scientific problem.
This paper evaluates the results of patients’ surgical treatment with the use of a combined mandibular
endoprosthesis consisting of individual titanium metal structure and titanium silk.
The authors analyze clinical and X-ray data of the postoperative period and give an objective assessment of
the technique effectiveness. Adequate restoration of the main functions of the lost organ is achieved due to the use
of a mandibular individual titanium endoprosthesis with included dental implants and manufacturing of an
orthopedic construction.
This solution provides reliable fixation by suturing soft tissue structures of the lower face area, including
facial and masticatory muscles, thus improving the biological integration of the endoprosthesis. The combined
design showed high efficiency of restoration of both aesthetic and functional characteristics of the mandible, as well
as good “survival” rate of endoprostheses in the long-term period of follow-up.
